Patient verification: Completed at visit start.
Geo-location verification: Captured at patient residence.
Subjective: Reports mild dizziness after lunch.
Objective: BP 108/68, pulse 74, no fall, gait steady.
Assessment: Possible transient postural drop.
Plan: Hydration encouraged, repeat BP in 2 hours, notify family contact if symptoms worsen.
